Orthodontic insurance is a big help.  We plan to help you receive your maximum benefit.  Most insurance companies only pay a portion of the orthodontic fee.  There are countless agreements between insurance companies and patients, and each contract provides a different benefit.  Payments from the insurance carrier will be sent directly to you.  We will make a payment schedule with you to pay the orthodontic fee.  We will help you claim your benefit from the insurance carrier.  Here is how we can work together to capture your maximum benefit.

 

What We Do:

Attending Dentist’s Statement:  This is the insurance claim form.  We will complete our portion, which will describe the diagnosis, treatment plan, and the payment arrangements.

 

What You Do:

If you would like our office to submit an insurance claim, please complete the insured or employee portion of your dental claim form, sign where indicated and submit to our office.  Please do not assign benefits to our office.  Benefits will be paid to the insured directly.  If you have questions about your benefits, please call your insurance company.  Note: Be sure that the form includes the name, address, and phone number of the insurance carrier.

 

During Treatment:

During treatment, some companies will require that you verify that treatment is continuing.  The company may provide a form for this, or request billing from us.  If you let us know which, it will help avoid a delay of your benefit check.
